RapidTesting Miami (RTM, LLC) · Version 2026-10-07 · Effective October 7, 2026. This page is part of our Terms of Service and Privacy Policy, and explains each choice on our clinic-finder form.

Only the first item is required to get your clinic results. Items 2–4 are optional, separate, and unchecked by default. Saying no never affects your results or access to any service.

1. Verification code and clinic results (required)

Program: RapidTesting Miami Clinic Finder. When you request clinic results, we text you a one-time verification code and, after you verify, a message with your nearest clinics (names, phone numbers and a booking link when available). Usually 2 messages per request. Message and data rates may apply. Reply STOP to stop, HELP for help, or call 855-560-2280. Carriers are not liable for delayed or undelivered messages.

2. Connect me with a clinic (optional)

If you choose this, we share your name (if provided), phone number, email, ZIP code, preferred language, age and the services you are looking for with the partner clinic you are matched to, so that clinic can contact you by phone, text or email to schedule a visit. We do not share your STD history under this option. Once shared, the clinic handles your information under its own privacy practices. You can withdraw this choice at any time (see section 5); it does not undo information already shared.

3. Offers and updates (optional marketing consent — adults 18+ only)

By choosing this, you give your prior express written consent to receive recurring marketing and promotional text messages, phone calls (including calls and texts sent using automated technology or prerecorded/artificial voice) and emails from RapidTesting Miami at the phone number and email you provided, about STD/HIV testing, PrEP, partner clinics, health programs, events and offers. Message frequency varies (typically up to 4 per month). Message and data rates may apply. Consent is not a condition of any purchase or service.

Payment disclosure: some of these messages promote our partner clinics, and RapidTesting Miami may receive payment from those partners for them. Reply STOP to any text to cancel, or use the unsubscribe link in any email.

4. Research opportunities (optional HIPAA authorization — adults 18+ only)

If you choose this, you authorize RapidTesting Miami to:

• Contact you about research studies, including paid sample-collection studies and clinical trials; and
• Share your name (if provided), phone number, email, age, ZIP code, preferred language and the health information you gave on the form (including prior STD history and your answers about research interest) with partnered clinical research facilities and their study sponsors, for the purpose of telling you about studies and checking whether you may qualify.

Expiration: this authorization expires 2 years after the date you gave it, or sooner if you revoke it. Revocation: you may revoke it at any time by emailing privacy@rapidtestingmiami.com or writing to our Privacy Officer; revocation does not affect information already shared in reliance on it. Voluntary: your access to clinic results and any service does not depend on this authorization. Re-disclosure: once shared with a research facility, your information may no longer be protected by federal privacy law (HIPAA). Payment disclosure: RapidTesting Miami may receive compensation from research facilities for referrals. You may request a copy of this authorization.

5. Your choices and how to reach us

• Unsubscribe from emails using the link in any email.

We do not sell your phone number, and we do not share mobile numbers or text-messaging opt-in data with third parties for their own marketing. Information is shared only as described in sections 2 and 4 when you choose those options, with service providers who help us operate (for example, our messaging and hosting providers), or as required by law.
