Your rights as a patient
Under HIPAA, you have the following rights regarding your protected health information (PHI):
- Right to access: You may request a copy of your medical records.
- Right to amend: You may request a correction to your records if you believe they are incorrect.
- Right to an accounting of disclosures: You may request a list of disclosures we have made of your PHI.
- Right to request restrictions: You may request that we restrict how we use or disclose your PHI for treatment, payment, or operations.
- Right to confidential communications: You may request that we contact you in a specific way (e.g., only by mail, only at a certain phone number).
- Right to a paper copy of this notice: You may request a paper copy at any time.
- Right to file a complaint: You may file a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated.
How we may use and disclose your PHI
We may use and disclose your PHI for the following purposes without your specific authorization:
- Treatment: Coordinating with partner clinics, research sponsors, and other providers to manage your care.
- Payment: Billing insurance or processing patient payments where applicable.
- Operations: Quality improvement, training, certification, licensing, accreditation, and credentialing.
- Required by law: Disclosures mandated by federal, state, or local law (e.g., communicable disease reporting).
For any other use or disclosure, we will obtain your written authorization. You may revoke that authorization at any time, in writing.
Our obligations
RTM, LLC is required by law to:
- Maintain the privacy of your protected health information
- Provide you with this Notice of our legal duties and privacy practices
- Follow the terms of the Notice currently in effect
- Notify you if a breach occurs that may have compromised the privacy or security of your PHI
Changes to this Notice
We reserve the right to change this Notice. Changes will apply to all PHI we maintain. Updated Notices will be posted on this page with a new effective date.
