How RTM, LLC handles your protected health information.

Your rights as a patient

Under HIPAA, you have the following rights regarding your protected health information (PHI):

Right to access: You may request a copy of your medical records.
Right to amend: You may request a correction to your records if you believe they are incorrect.
Right to an accounting of disclosures: You may request a list of disclosures we have made of your PHI.
Right to request restrictions: You may request that we restrict how we use or disclose your PHI for treatment, payment, or operations.
Right to confidential communications: You may request that we contact you in a specific way (e.g., only by mail, only at a certain phone number).
Right to a paper copy of this notice: You may request a paper copy at any time.
Right to file a complaint: You may file a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated.

How we may use and disclose your PHI

We may use and disclose your PHI for the following purposes without your specific authorization:

Treatment: Coordinating with partner clinics and other health care providers involved in your care.
Payment: Billing insurance or processing patient payments where applicable.
Operations: Quality improvement, training, certification, licensing, accreditation, and credentialing.
Required by law: Disclosures mandated by federal, state, or local law (e.g., communicable disease reporting).

For any other use or disclosure, we will obtain your written authorization. You may revoke that authorization at any time, in writing.

Uses that require your written authorization

We will not use or disclose your PHI for the following purposes unless you give us your written authorization:

Marketing: communications that encourage you to use a product or service, including messages for which RTM receives payment from a partner clinic or other third party. Our authorization will tell you when payment is involved.
Research recruitment: sharing your contact information or health information with research facilities or study sponsors so they can determine whether you may qualify for a study.
Sale of PHI: we do not sell PHI; any such disclosure would require your authorization.

On our website, these authorizations are the optional “Offers and updates” and “Research opportunities” choices, described in our Messaging Terms & Consent Details. They are never required to receive services. You may revoke an authorization at any time by writing to privacy@rapidtestingmiami.com; revocation does not affect disclosures already made in reliance on it. Information disclosed under an authorization may be re-disclosed by the recipient and may no longer be protected by HIPAA.

Our obligations

RTM, LLC is required by law to:

Maintain the privacy of your protected health information
Provide you with this Notice of our legal duties and privacy practices
Follow the terms of the Notice currently in effect
Notify you if a breach occurs that may have compromised the privacy or security of your PHI

Changes to this Notice

We reserve the right to change this Notice. Changes will apply to all PHI we maintain. Updated Notices will be posted on this page with a new effective date.
